What this website collects

The appointment request form asks for your name, phone number, email address, the type of visit you want, a preferred day and time window, and a short description of the problem. Nothing else is requested and no account is created.

  • We use those details only to contact you about scheduling.
  • The form does not process payments and never asks for insurance ID numbers, a Social Security number or card details.
  • Please keep the description brief — a website form is not the place for a full medical history.

How requests reach us

A submitted request is sent to the clinic inbox and the front desk calls you back, normally the same working day. Requests are kept only as long as needed to schedule the appointment; once you become a patient, your information lives in the clinical record instead.

Cookies and analytics

This site sets no advertising cookies and runs no third-party tracking pixels. Web fonts are loaded from Google Fonts, which means your browser requests those font files from Google's servers. Standard server logs (IP address, page requested, timestamp) are kept briefly for security and troubleshooting.

Sharing

We do not sell, rent or trade information from this website. Clinical information is shared only where care or law requires it — for example with your referring physician or surgeon, or with your health plan when we file a claim on your behalf.

Your choices

  • Ask us to delete an appointment request that has not become an appointment.
  • Ask for a copy of, or a correction to, information we hold about you.
  • Tell us how you prefer to be contacted — call, text or email.

Accessibility and site changes

If any part of this site is difficult to use with assistive technology, call the clinic and we will take the information over the phone and fix the page. This policy may be updated; the date at the top always reflects the current version.
